A patient is given a questionnaire card when arriving at the front desk.

After filling out the card, the patient gives the card back to the front desk clerk who uses PatientLink and an Optical Mark Reader (OMR) to read the card and send the values into the EMR.

The values are written to the database where they can then be viewed by the clinician and become part of the documentation for the office visit.
